Summary: Primarily responsible for providing evidence-based psychotherapy, counseling, and psychoeducational services at Crescent Community Health Center (CCHC). Assess, evaluate, diagnose, and treat signs/symptoms related to disorders outlined in the most recent version of the Diagnostic and Statistical Manual of Mental Disorders (DSM) across the lifespan. Actively engage and coordinate with all members of the patients integrated care team (care management, medical, dental, brain health, etc.).
Essential Duties and Responsibilities:
- Perform all job functions according to CCHC mission and values.
- Provide biopsychosocial assessments of patients and families to diagnose and identify emotional, social, and environmental strengths and problems related to their diagnosis, condition, treatment, and/or life situation.
- Develop, implement, and manage a comprehensive treatment/care plan utilizing evidence-based clinical/therapeutic interventions to treat and address an individuals identified needs including but not limited to brain health and physical health.
- Provide immediate brief intervention and/or crisis intervention services related to an individual's brain health as required or needed.
- Provide education to patients and families around issues related to the patient's diagnosis, condition, treatment and/or life situation.
- Accurately complete all patient records and documentation necessary for timely billing of services.
- Maintain confidentiality around patient and family protected health information.
- Work as a member of the patients care team to establish effective relationships with colleagues, patients, referral sources, and others involved in the patients care to provide patient-centered integrated services.
- Participate in team huddles and staff trainings/conferences as requested or assigned.
- Participate in internal and external chart reviews for ongoing professional development and performance evaluation.
- Other related and/or necessary tasks as assigned.
Qualifications:
- Minimum of a master's degree from a nationally accredited institution or from a program approved by the appropriate licensing board in the State of Iowa for social work, behavioral health/sciences, or other related field, required.
- Licensed in the state of Iowa as an LMSW, LISW, tLMHC, LMHC, tLMFT, LMFT, or LBA, required.
- Ability to act independently, respond professionally and calmly in crisis, urgent, or emergent situations, required.
- Communicate effectively both verbally and in writing, required.
- General knowledge of risk reduction, brain health disorders (substance use, mental health), psychotherapy, and psychopharmacology, preferred.
- Minimum of 2 years' experience in a health care or social service setting, preferred.
Other: This position requires a valid driver's license, verification of personal auto insurance and the ability to become an authorized driver as detailed in the driver authorization procedure and the ability to accommodate routine local travel and occasional travel outside of the local area on behalf of CCHC.
Pre-employment drug testing, physical, review of required immunizations including COVID Vaccination and background check required.
