The City of Cascade (population 2,104) is located in rural Dubuque and Jones Counties sitting along Highway 151, 25 miles Southwest of Dubuque. The City of Cascade operates under a City Administrator form of government with a Mayor and five City Council members. The City has 9 full-time employees and a number of part time employees as well. The City also has an Electric and Gas Utility that employs 5 full-time employees.

CLC currently provides community supported living services to approximately 200 individuals who live throughout Dane County, primarily in Madison, and Stoughton. We are excited to be expanding to serve individuals in the communities of Platteville, Beaver Dam and Fond du Lac.

Our organization provides support that enables people to live in their own homes and participate in community life. We believe that individuals, regardless of ability, deserve equal opportunities to create their “best life”; they just need additional support in order to do so. Community Living Connections provides in home staff to help with planning, personal cares, medical support, finances, relationship buildingΓǪetc. We work in partnership with people throughout their lifetime.
