Job Overview

Summary: Under the supervision of the Director of Brain Health (or designee), the BHSI will provide direct and indirect support services to aid CCHC staff in providing integrated brain health services to CCHC patients. Primary role will be to provide education, screening, and needs assessment to determine the type and level of services needed. They will attempt to support patient engagement through regular patient contact via phone, real-time audio/video, and/or in-person contact. This is for a fall 2025 internship.

Essential Duties: Essential volunteer duties include direct and indirect patient contact, ongoing development through formal (weekly) supervision, and the completion of all required internship hours/requirements as outlined within the practice site agreement. Other duties must be performed as assigned or required.

  • Incoming Referrals:
  • Explain integrated services (active involvement in both medical and BH care), including what it means to be an active BH patient as outlined in policy #80-03.
  • Screen for the desired services, reason for the request, and the referral source to determine potential eligibility (scope of practice, current status with CCHC) as outlined in policy #41-27.
  • Based on screening results, coordinate with appropriate CCHC staff to either refer to an external provider (not accept at CCHC), schedule with the appropriate provider, and/or transfer to BH RN or appropriate care team to triage immediate needs and/or questions.
  • Coordinate with the medical scheduler to schedule individuals that want to establish medical care with CCHC as part of our integrated approach.
  • Environment/Behavior:
  • Perform all functions according to CCHC's mission/vision/core values, established policy/procedure, and confidentiality standards (HIPAA).
  • Treat all staff with respect and communicate using various methods of communication (email, Teams, phone, in person).
  • Maintain a clean, organized, and safe environment for others, including safeguards related to protected health information.

Expectations:

  • Communicate with the primary site supervisor as it relates to program assignments, reviews, and evaluations in a timely manner.
  • Effectively communicate needs (hours, skill development, supervision, etc.) along with any changes in internship/practicum schedule (start/end dates, hours per day, etc.).
  • Track and monitor all required hours in order to comply with the practice site agreement as CCHC is not responsible for tracking and/or insuring practice site requirements are met.
  • Maintain established policy/procedure, workflows/procedures, and confidentiality of CCHC's patients and employees.
  • Contribute to the success of the clinic by promoting CCHC's mission, vision, and culture (teamwork, openness, respect).

Qualifications: The individual in this position must be a graduate level student seeking a masters degree in therapy or social work or other related field.

Education/Experience:

  • Pursuing a Masters Degree in Social Work, Mental Health Counseling, Marriage and Family Therapy or other related field.
