TITLE: Certified Advanced Practice Social Worker (CAPSW) or Licensed Clinical Social Worker (LCSW) / Counselor

POSITION STATUS/HOURS: Part-time .8 FTE (32 hours/week) Monday – Friday. Three days per week in the clinic providing counseling services and one day per week in the hospital providing social work Benefit eligible position!

 

JOB SUMMARY

The role of the hospital social worker is to evaluate patients, determine their needs, and put together action plans to make sure those needs are being met efficiently during emergency room, inpatient, observation, OB and swing bed stays. This includes ensuring that the patients’ welfare is protected along with assisting to plan appropriate discharge. The social worker provides support services to patients while they are in the hospital and works in collaboration with the care team to arrange for continued help and care when the patient is discharged. This role will also include providing mental health individual and family counseling sessions to patients of varying ages in the Rural Health Clinic.

 

SOCIAL WORKER RESPONSIBILITIES

•    Perform psychosocial assessments on Medicare, Medicaid, and high-risk patients to determine social service and discharge planning needs, and document these assessments

•    Collaborate with Utilization Review Case Manager and other members of the health care team to provide for psychosocial assistance or optimum post-discharge services

•    Communicate with physicians and medical providers regarding social service and discharge planning needs of patient. Assist the physician to facilitate post-hospital care

•    Maintain current knowledge of resources available within the community; and maintain supply of resource materials to be distributed to patients when needed. Obtain other resources as needed

•    Communicate with family members and caretakers regarding the needs of the patient and anticipated plans

•    Attend daily care conference

•    Assist patient/family with the grieving process making referral to pastoral care, as appropriate

•    Demonstrate advocacy toward patient and family related to: advance directives; DNR/resuscitation plan; discharge plan; liaison between physician, patient/family, staff; participates in achieving department goals

•    Consult with emergency department patients and OB patients as requested

•    Consult with patients regarding financial concerns as requested

•    Assist patients with completion of advance directives

•    Coordinate patient disposition

•    Conduct referral assessments

•    Report child abuse and elder abuse/neglect

•    Assist with any needs of the Ethics Committee

•    Work in collaboration with Compliance to coordinate efforts across language and cultural barriers for effective communication, including staff education

•    Address and support cultural practices as long as such practices do not harm others or interfere with the planned course of medical therapy

•    Demonstrate a high degree of accuracy and attention to details to optimize the cost-effective delivery of care

•    Assist quality resources department with projects as needed

 

COUNSELOR RESPONSIBILITIES:

•    Act as a catalyst for mental wellness, empowering individuals and families as a provider of mental health counseling

•    Utilize your passion to cultivate health with your career and identify clinical criteria (DSM-5/ICD-10) for diagnosis and treatment of children, youth, adolescent/young adult, adult populations and couples and families seeking counseling

•    Apply evidenced-based, innovative therapeutic modalities to treat patient's presenting disorders and increase symptom alleviation

•    Develop treatment plans that align with client strengths and challenges, with emphasis on treatment modalities that are a best fit to client’s DSM-5 diagnosis

•    Complete timely documentation in accordance with state and ethical guidelines

•    Work in collaboration with providers to promote overall health of patients who are referred for mental health services

 

QUALIFICATIONS:

1.    Master’s Degree in Social Work or PhD in Psychology

2.    Current Wisconsin licensure as a Licensed Clinical Social Worker (LCSW), or Certified Advanced Practice Social Worker (CAPSW), or a Psychologist
