Overview

We are looking for a full-time Homecare Aide to join our Dubuque Visiting Nurse Association. Work hours will be days, Monday - Friday, for 80 hours per pay period, and as needed. This position doesn't work nights or weekends.

The VNA Home Care Aide position works directly within the VNA Public Health services grant to assure the provision of core public health functions and essential public health services. VNA homemakers support the improvement of local public health capacity to address health priorities and develop conditions for healthy people and healthy communities through the provision of support services driven by patient needs.

Responsibilities

Home Aide Services

  • Performs personal cares as directed by patient-driven care plan. Including bathing, grooming, oral health, shaving, nail care dressing.
  • Provides homemaking services as directed by patient-driven care plan. Including: vacuuming, dusting, laundry, dishes, mopping, and other light maintenance tasks.
  • Notifies office staff of patient requests and scheduling concerns.
  • Documents work time, mileage, and services provided accurately and timely; including self-scheduling and patient volume reporting.
  • Establishes and maintains effective working relationships within the department and with external contacts.
  • Provides patient supervision and companionship when assigned for non-personal care respite services
  • Provides admitting and recertification services on patients enrolled.

Home Care Aide

  • Catheter care, and colostomy care as needed.
  • Performs physical activities as directed by patient-driven care plan, including patient transfer, walking/ambulation, and hoyer.
  • Performs special services as directed by patient-driven care plan, including temperature-check, pulse-check, and blood pressure-check.

Basic UPH Performance Criteria

  • Demonstrates the UnityPoint Health Values and Standards of Behaviors as well as adheres to policies and procedures and safety guidelines.
  • Demonstrates ability to meet business needs of department with regular, reliable attendance.
  • Employee maintains current licenses and/or certifications required for the position.
  • Practices and reflects knowledge of HIPAA, TJC, DNV, OSHA and other federal/state regulatory agencies guiding healthcare.
  • Completes all annual education and competency requirements within the calendar year.
  • Is knowledgeable of hospital and department compliance requirements for federally funded healthcare programs (e.g., Medicare and Medicaid) regarding fraud, waste and abuse. Brings any questions or concerns regarding compliance to the immediate attention of hospital administrative staff. Takes appropriate action on concerns reported by department staff related to compliance.

Qualifications

Minimum Requirements

Preferred or Specialized

Not required to perform the essential functions of the position.

Education:

· High School diploma or GED.

· Completion of a nursing assistant program with a minimum of 75 hours with no continuous period of 24 months without compensation for home care aide/nursing assistant work since completion.

Experience:

One year of prior experience working with older adults in a public health care setting

License(s)/Certification(s):

· Valid certification, if required by state(s) where employed and providing care.

· CPR Certification.

· Valid Mandatory Reporter course completion by state(s) requirement.

· Valid, licensed driver, with automobile insurance in accordance with state and/or organizational requirements.

CNA certification

Knowledge/Skills/Abilities:

· Strong interpersonal skills.

· Proficiency with computer applications.

· Ability to work as a collaborative team member across service lines.

· Ability to understand and apply guidelines, policies and procedures.

· Maintains a reliable mode of transportation and has the ability to drive safely during all day and night hours and in all types of inclement weather.

Other: Continuing Education Requirements

· A minimum of 12-hours of age-specific in-service training under the supervision of a RN annually. Content must include:

o Emergency Preparedness Plan

o How to handle patient and caregiver complaints/grievances

o Infection control

o Cultural diversity

o Communication barriers

o Ethics training

o OSHA, workplace and patient safety

o Patient rights and responsibilities

o Compliance program

· CPR: Maintain a valid Basic Life Support (BLS) Healthcare Provider Card with Re-certification.

· Mandatory Reporter Education (per state regulation).
