Job Overview

We are looking for a dynamic community/clinical pharmacist to join our team Part time at Crescent Community Health Center (CCHC) and InFocus Pharmacy Services. We are looking for a part-time pharmacist to work two 10 hour days with NO nights, weekends, holidays, or on-call. Crescent provides a diverse, fun, inclusive workforce with competitive wages and benefits.

You will be joining an enthusiastic group of pharmacists and technicians and be a part of a medical practice which includes acute-care and ambulatory services. At CCHC, we strive to base our practice on the NIH Quadruple Aims, which was developed to put the patient back in focus, reducing costs and improving health outcomes while preventing burn-out in health care providers. Currently we have an established collaborative practice and our clinical pharmacists help manage diabetes, anticoagulation, polypharmacy and other complex medication regimes. As a pharmacist at InFocus and CCHC you will have access to the EHR, Epic, for critical patient health information and to communicate with other healthcare providers in the building. We are also looking forward to implementing the new Iowa Standards of Care Practice Standards.
