Job Description

Operation Empower is seeking a compassionate and dedicated Case Manager to join our team in a full-time role (40 hours per week). This vital position offers the opportunity to make a meaningful impact by providing individualized case management, counseling, and support services to adults in recovery. The Case Manager will assess client needs, develop tailored recovery plans, and coordinate with community resources to support clients’ mental health, sobriety, and reintegration into the community.

Responsibilities include leading on-site recovery groups, assisting clients in accessing wraparound services, advocating across systems of care, and ensuring smooth transitions into our recovery community. The role also involves using evidence-based approaches, fulfilling administrative duties, and maintaining professional and ethical standards.

Other Responsibilities Include

  • Maintain a client caseload, including eligibility assessments and individualized recovery planning
  • Conduct initial needs assessments and collaborate with supervisors on case management strategies
  • Track and document client progress through accurate case notes and timely reporting
  • Maintain regular communication with clients, offering flexible meeting times as needed
  • Build and maintain strong relationships with community partners and service providers
  • Participate in staff meetings, training sessions, and community outreach
  • Perform other duties as assigned to support program operations
  • Coordinate with community resources (e.g., recovery groups, mental health services, housing programs) to support client goals

Requirements and Skills

  • Bachelor’s degree in Social Work, Psychology, Human Services, or related field; Master’s degree preferred
  • Valid driver’s license for transporting clients when necessary
  • Strong oral and written communication skills
  • Ability to work collaboratively as part of a multidisciplinary team
  • Physical ability to lift up to 30 lbs. and assist with client-related activities as needed
  • Flexibility to adapt to program needs and client schedules
  • Commitment to complete Dependent Adult Abuse Mandatory Reporter Training within 6 months of hire (and every 5 years thereafter)
