Job Purpose:
- The Community Health Worker at Crescent Community Health Center will serve as a vital liaison between the community and healthcare providers. The role is designed to enhance access to healthcare services, improve health outcomes, and promote health education within the community. The Community Health Worker will work collaboratively with healthcare professionals to identify community needs and develop strategies to address them effectively.
Key Responsibilities:
- Conduct outreach activities to engage community members and promote health services offered by Crescent Community Health Center.
- Educate community members on health topics, disease prevention, and healthy lifestyle choices.
- Assist individuals in accessing healthcare services, including scheduling appointments and providing transportation support when necessary.
- Collaborate with healthcare providers to identify and address barriers to healthcare access within the community.
- Advocate for community health needs and work to develop programs that address those needs.
- Maintain accurate records of community interactions and services provided, ensuring confidentiality and compliance with health regulations.
- Participate in community events and health fairs to promote Crescent Community Health Center services and initiatives.
- Provide support to healthcare providers in managing patient care and follow-up activities.
- Continuously assess community health trends and report findings to healthcare providers to inform service delivery improvements.
Required Education:
- High school diploma or equivalent required.
- Additional certification or training in community health, public health, or a related field preferred.
Required Experience:
- None; Minimum of 1-2 years of experience in a healthcare setting or community outreach role highly desired.
- Proven experience working with diverse populations and understanding community health needs.
- Care coordination experience in the healthcare or human services fields. Familiarity with local healthcare resources and services.
Required Skills and Abilities:
- Strong communication skills, both verbal and written, with the ability to convey health information clearly.
- Excellent interpersonal skills to build relationships with community members and healthcare providers.
- Ability to work independently and as part of a team, demonstrating initiative and problem-solving capabilities.
- Proficiency in using computer applications, including Microsoft Office Suite and electronic health records.
- Cultural competency and sensitivity to work effectively with individuals from various backgrounds.
- Ability to manage time effectively and prioritize tasks in a dynamic environment.
