Job Overview

Job Purpose:
- The Pharmacist at Crescent Community Health Center plays a critical role in advocating for safe prescribing practices and ensuring the accurate transcribing, dispensing, monitoring, and documentation of pharmacy activities. This position is essential in maintaining the integrity of the medication use system and contributing to the overall health and well-being of the community we serve.

Key Responsibilities:
- Advocate for safe prescribing practices to ensure patient safety and optimal therapeutic outcomes.
- Accurately transcribe and dispense medications while maintaining meticulous documentation of all pharmacy activities.
- Monitor patient medication regimens and provide guidance to healthcare providers and patients to optimize medication therapy.
- Procure medications and health-related items, ensuring that all inventory is stored properly and securely.
- Maintain the medication use system, ensuring compliance with all regulatory requirements and best practices.
- Participate in continuing education and professional development, including completing the 340B university full-day program within the first year of employment.
- Collaborate with healthcare providers to provide comprehensive pharmaceutical care and support to patients.
- Contribute to the development and implementation of pharmacy policies and procedures to enhance the quality of care provided by Crescent Community Health Center.
