Job Overview
Luther Manor Communities is seeking an energetic, knowledgeable team player to join our social services department.
POSITION: Responsible for helping to meet psychosocial needs of the facility residents. Responsible for maintaining quality mix and full census on the nursing home, and the completion of Medicare and State required assessments and other social services related tasks.
POSITION QUALIFICATIONS:
- A bachelor's degree in social work or a bachelor's degree in a human services field including, but not limited to, sociology, special education, rehabilitation, counseling and psychology.
- One year of supervised social work experience in a health care setting working directly with individuals.
- Previous supervisory experience preferred-
- Previous experience working with long term care admissions preferred.
- Regular attendance is required.
- Must be able to read, write and speak fluent English.
- ESSENTIAL DUTIES AND RESPONSIBILITIES:Resident Care:
- Assist each resident to adjust to the social and emotional aspects of illness, treatment and stay at the facility
- Relate effectively with residents, their families and staff and be knowledgeable of community agencies and resources available to help meet those needs.
- Meet with residents periodically to provide assistance in obtaining physical needs or addressing psychosocial needs such as coping with illness or grief, adjusting to new surroundings and situations, and other concerns such as adjusting to placement.
- Assist residents and families in utilizing community resources when not provided by the facility.
- Serve as the liaison with Resident Council and act as advocate for Resident Rights by educating residents of these rights, and other issues that may concern them.
- Assist with other disciplines in the total care of the residents through consultation, communication, and serving on committees as appointed.
- Be involved in any resident complaint investigations (e.g. missing items, complaints regarding other residents, dependent adult abuse, etc.)
- Assist in the completion of routine and unscheduled required cognitive, mood and behavioral assessments.
- Complete required care plans and assist with creating interventions to assist residents with adjustment to nursing facility care or to meet other psycho-social needs.
- Initiate and maintain long term care insurance claims on behalf of residents and families.
Admissions
- Fields calls, inquiries regarding admissions to appropriate levels of care (including PASARR).
- Give tours of the facility, explain admission procedures, give or mail admission packets to prospective resident and/or family.
- Review prospective new residents with Administrator / Director of Nursing
- Maintain waiting list.
- Contact resident/family when there are openings.
- Process the admission paperwork at the time of admission.
- Ensures PASARR admission requirements are met.
- Perform insurance verification and pre-authorizations (when applicable) for all admissions.
- Conduct admission meetings weekly or as needed.
- Inform staff that need to know of new resident's room number, time of arrival, pay source, etc.
- Maintain good working relationships with referral sources.
- Act as a supplement to the Director of Social Services, providing assistance with admissions and referrals when the Director is unavailable.
- Review all referrals with the Director of Social Services.
- Help to maintain good working relationships with referral sources.
Care Conference & Medicare:
- Assist in the completion of the resident's Minimum Data Set Assessment related to mood, behaviors and psychosocial needs. This shall be done upon admission, quarterly or whenever a significant change in condition occurs.
- Be an active member of the Interdisciplinary Care Conference Committee by:
- Encouraging residents and families to participate in Care Plan Conferences.
- Attend Care Plan Conferences as scheduled.
- Develop goals based on resident needs as identified in assessments, interviews and observations.
- Communicate with other members of the Care Conference Committee to coordinate goals.
- Serve as a team player with focus on the welfare of the resident and providing optimal care.
- Maintain resident records in "Social Service" section of resident chart to include progress note entries, social history, social service assessment and any other pertinent information.
- Progress notes will be done at least quarterly and will reflect the plan of care and overall functioning of the resident.
- Be an active member in weekly Medicare meetings
- Familiar with Medicare skilled levels of care regulations
Professional Duties
- Represents the facility and the Social Services/Admissions Department in a professional manner within the community.
- Works appropriate hours as needed. Is accountable for his/her time/work responsibilities.
Complete requirements for in-service training. . Regular attendance is required.
