Summary
The Social Worker applies social work methods to engage, educate and empower Crescent's patients and the communities we serve. . This individual will work as part of a care team assisting clients/patients in navigating a variety of systems within and outside of Crescent to overcome barriers pertaining to access to healthcare.
Essential Duties
- Assists in developing and implementing social service programs and resources.
- Uses evidence-based practice to assess and resolve complex social, economic, and psychosocial barriers that affect care such as motivational interviewing, SBIRT, PHQ-9.
- Completes PRAPARE assessments and makes referrals according to needs identified.
- Provides referral coordination with community resources and agencies.
- Assists patients with applications for programs or benefits specific to their needs i.e., Medicaid, Health Home, employment opportunities, etc.
- Collaborates with providers and specialists to obtain records, coordinate referrals, and ensure continuity of care.
- Communicates referral status and information to patients in a timely manner.
- Connects patients to behavioral health or substance abuse treatment services, and other community resources to ensure that the patient is accessing what he/she needs to improve or maintain health status.
- Arranges transportation for patients when identified as a barrier to care. Tracks and reports approved transportation coverage specific to budgetary and other grant requirements.
- Maintains a professional, collaborative work environment to foster a positive public image for Crescent CHC.
- Provides crisis intervention as needed.
- Reports cases of abuse and neglect to DHS according to the mandatory reporter standard for relevant cases
- Meets with the Patient Care Team in the daily huddle in order to communicate necessary information regarding patient visits.
- Prepares requested reports.
- Assists in the collection of key data points in order to track quality benchmarks.
- Places outreach and follow-up calls to assigned patients and tracks calls made.
- Tracks patient compliance with recommended care and referrals.
- Provides evidence-based educational materials to the patient as directed by the provider or other care team members to empower the patient with self-management of their illness.
- Updates patient charts in EHR according to specialty care visit notes.
- Tracks patients in i2i and EHR to manage patient populations, including vulnerable and high-risk patients and patients needing a visit.
- Answers and returns phone calls to patients in a timely manner.
- Acts as a liaison with other departments within Crescent CHC.
- Performs quality assurance and utilization management audits of patient charts to assure compliance with various program expectations.
- Advocates, connect resources, and educates at the community level to inform the general public about the Crescent's offerings and the special populations served.
- Works toward continuous quality improvement.
- Stay current with changing technology, including software and programs.
- Uphold, support, and promote all company policies and procedures.
- Establishes boundaries and maintains a code of ethics according to social work standards.
- Completes other duties as assigned.
Qualifications
- Bachelor's degree (B. A.) in social work or related field from four-year college or university
- Prior related experience preferred.
- Must hold current certification as a mandatory reporter or be willing to obtain within 3 months of hire.
Pre-employment drug testing, physical, review of required immunizations, and background check required.
