Job Overview

Why Crescent Community Health Center? Crescent is a mission driven, patient centered organization. Crescent provides medical, dental and brain health services to the Greater Dubuque area including our most vulnerable populations. Crescent works hard to bring high quality affordable health care to all in our community. Crescent provides a diverse, fun, and inclusive workforce with competitive wages and benefits. If you are looking for a rewarding career that makes a difference in the community, this is your chance. We are seeking someone who wants to make that difference in our InFocus Pharmacy as one of our dispensing pharmacists.

You will advocate for safe prescribing practices and accurate transcribing, dispensing, monitoring and documentation of pharmacy activities. You will also be responsible for procuring medications and health-related items, ensuring proper storage, and maintaining the medication use system.

Our pharmacists ensure adherence to the policies and procedure at the facility and adherence to state and federal regulations. You will also need to be able to communicate with medical professionals about patients and their medications, listen to patients' reports of symptoms and provide suggestions for over-the-counter medications.

You will also safely administer vaccines, performing CLIA waive tests, and serving as point of escalation for patient, provider, or customer concerns.

You'll also need to be able to collaborate with both patients and providers to help with medication reviews, disease and adherence management and any patient support services. You are also critical in helping patient care teams with high-risk populations. You will need to ensure you maintain adequate training and pharmacist certifications to support these programs.

Successful applicants will have a BS or PharmD obtained from an accredited School of Pharmacy, prior retail pharmacy experience, and be licensed with the Iowa State Board of Pharmacy and will seek licensing within other states Crescent operates, within a year. You will also need to complete an in person, 340B university full-day program within the first year of employment. We'd prefer you to have completed an ASHP accredited residency program, and bi-lingual language ability in either Marshallese or Spanish is a plus.

This is a full-time, 40 hour per week role.

Please see the full job description for more detail. Background check, pre-employment physical, TB test, drug screen and credentialling required.
