Job Overview
Your Responsibilities
As IHH Nurse Case Manager, your primary responsibility includes planning, directing, coordinating and providing whole health services to the clients of Integrated Health Home (IHH) Program. In addition, you will:
- Reviewing medical records to provide information, guidance, and recommendations for treatment plans
- Participating in care coordination activities and case discussions for members
- Promoting access to behavioral health and physical health services and collaborates between these entities when needed
- Understanding and monitors each IHH member’s concerns and the types of whole health resources needed
- Communicating the needs for health services to clients and staff
- Other duties as assigned
What You Need
- RN or BSN with current Iowa licensure in good standing
- At least one year experience working with persons with mental illness
- Knowledgeable on psychotropic medications, DSM-IV diagnoses, co-occurring disorders and be able to work amongst an interdisciplinary team.
- Valid driver license and ability to meet agency driving requirements.

About the Program

- Treatment of particular mental illnesses including anxiety, depression, bipolar syndrome and ADHD
