Unified Therapy Services is hiring an Occupational Therapist to provide services at our pediatric therapy clinic in Lancaster, WI. Unified Therapy Services strives to provide the highest quality of care to all individuals. At our four clinic locations, we believe in a collaborative approach between our occupational and physical therapists and speech-language pathologists. Our therapists pride themselves on their skill sets and ongoing certifications and training.

Occupational Therapist Job Summary

  • Assesses patient and determines treatment plan using approved evaluation documents
  • Understands and complies with universal precautions, safety and risk policies and procedures
  • Prepares necessary documentation including evaluations, progress notes, discharge notes and logs in timely manner according to regulations
  • Consults with other members of rehabilitation team to select treatment programs consistent with needs and capabilities of individual and to coordinate comprehensive services delivery
  • Trains individuals in skills and techniques required for participation in ADL and IADL
  • Responsible for supervision of a COTA
  • Designs and constructs adaptive equipment for individual and recommends adaptation of individual’s work-living environment to maximize independence
  • Provides services to pediatric and UE orthopedic patients
  • Maintains positive relationships with the health care team, patient and family

Clinic Hours of Operation

  • Monday-Thursday 8am-6pm

Unified Therapy Services is an outpatient clinic with three locations in Dubuque, Iowa and one in Lancaster, WI. We offer physical, occupational, behavioral and speech-language therapy services to adult and pediatric patients in the tristate area.

All clinic locations have private treatment rooms as well as open gym areas to accommodate all appointment types. Our pediatric clinics have indoor playgrounds as well as an abundance of swings, games and toys to keep therapy engaging and fun!
