Description:
POSITION PURPOSE:
Responsible for ensuring the safe, appropriate, and cost-effective use of anti-cancer and oncology/hematology supportive care medications. Provide expertise on drug therapy initiation and monitoring to all customer groups served by MercyOne Cancer Center, working collaboratively with other health care professionals and patients to devise the most appropriate drug treatment regimens. The role of the clinical pharmacy specialist sometimes extends outside the Cancer Center, so he/she can share their specialized knowledge with other network hospitals, clinics, and care settings. He/she shall also be able to function as an inpatient staff pharmacist and provide pharmaceutical care for all populations within the hospital.

What you will do:
- Demonstrates specific customer focused behaviors.

- Acts in a professional manner reflecting the mission, vision, and values of MercyOne.
- Reviews medication orders to determine appropriateness of therapy including, but not limited to, dose, dosage form, route of administration, schedule, starting time, and duration.
- Monitors patient medication regimens for possible allergies, drug interactions, nutrient-drug interactions, contraindications, and potential adverse drug reactions.
- Individualizes medication therapy based on patient variables including, but not limited to, renal function, age, weight, laboratory values, and pharmacokinetic parameters.
- Manages drug therapy and utilization across the continuum of care, documents clinical interventions, participates in quality assurance monitors, drug utilization review, and pharmacoeconomic analysis, and monitors medication therapy outcomes.
- Consults with and advises providers in the development of safe, rational, and cost-effective patient medication care plans.
- Responds promptly and thoroughly to requests from healthcare personnel for patient-specific pharmacotherapeutic information.
- Directs and monitors pharmacy technicians in preparation, packaging, labeling, and distribution of medication.
- Directs, monitors, and verifies the compounding of injectable medications, including epidural infusions, irrigations, and small and large volume parenterals performed by pharmacy technicians.
- Compounds and dispenses medications and injectables (including cytotoxic and hazardous drugs); according to established policies, procedures, State and Federal regulations.
- Provides and prepares emergency medications in emergency carts, kits, and EMS/Air Care kits.
- Maintains the confidentiality of information acquired pertaining to patients, physicians, employees, and visitors to MercyOne Dubuque Medical Center. Discusses patient and hospital information only among appropriate personnel in appropriately private places.
- Maintains current knowledge of departmental policies and procedures as well as state and federal regulations and maintains any documentation required by the same.
- Assumes responsibility for performance of job duties in the safest possible manner to assure personal safety and that of coworkers, and to report all preventable hazards and unsafe practices immediately to management.
- Takes an active role in enhancing abilities to carry out job functions through personal and professional/job-related growth and development.
- Assists with Cancer Center Pharmacy operations
- Completes all expected pharmacy functions, duties, and responsibilities prior to leaving assigned shift/period and work area. This includes using time at work in a productive manner.
- Keeps accurate and legible records and documentation forms.
- Performs duties in a cost-effective manner, striving to avoid wasting resources. This includes using time at work in a productive manner, providing/implementing suggestions, and documenting activities that conserve resources (financial and human).
- Serves as a member of the Pharmacy and Therapeutics Committee for oncology/hematology drug review.
- Maintains professional education/development and assists in the education of other team members.
- Supports and abides by all medical center, departmental, and safety policies and procedures. Proactively identifies safety concerns surrounding the work environment.
- Performs other responsibilities as requested by the pharmacy director or supervisor.
What we are looking for:
- Requires a Bachelor Degree in Pharmacy. A PharmD or other related advanced degree is preferred.
- Iowa pharmacist licensure, either by examination or reciprocity is required.
- PGY2 Oncology residency training is required or equivalent clinical experience in oncology setting.
- Board of Pharmacy Specialties Oncology certification is desirable within 24 months of hire.
