Job Overview
Consistent with the American Nurses Association Scope and Standards of Practice (2004), the Patient Care
Coordinator is a professional nurse and a member of the Mercy Medical Center healthcare team and is
accountable for the delivery of holistic, compassionate, individualized, and effective patient care demonstrated by
the use of the nursing process: assessment, diagnosis, outcome identification, planning, implementation, and
evaluation. In addition, the Patient Care Coordinator is responsible for behaviors consistent with the professional
nursing role, appropriate to their education and experience, including activities related to quality of care,
performance appraisal, education, collegiality, ethics, collaboration, research and evidence-based practice, and
resource utilization. The Patient Care Coordinator facilitates interdisciplinary team planning to maximize patient
outcomes throughout the continuum of care and serves as a clinical resource for the defined patient population.
The Patient Care Coordinator assumes accountability for the nursing department in the absence of the
Director/Supervisor.
The Patient Care Coordinator will complete and maintain competencies to provide care within the assigned unit:
● Core competencies for all RNs
● Basic and advanced competencies for the specified department:
- Conflict mediation
- Cost containment
- Delegation
- Health system guidance
- Incident reporting
- Laboratory data interpretation
- Physician support
- Staff development
- Staff supervision
- Team work support
The Patient Care Coordinator must be a registered nurse and must hold an active license to practice nursing in the state of Iowa. A baccalaureate degree in nursing and certification by a nationally recognized nursing organization is preferred and strongly encouraged. An applicant must have a minimum of an associate’s degree in nursing. The registered professional nurse must continuously seek opportunities to enhance their own professional nursing practice. Membership in an appropriate professional nursing organization is recommended and previous leadership experience including charge experience, community leadership, group facilitation, committee or project involvement or an active role in the change process is necessary. The job requires a high degree of interpersonal skills to deal with a range of complicated problem situations.
