Position Summary

A Dubuque Pharmaceutical company is seeking a Patient Care Coordinator to join their team. As Patient Care Coordinator, you will be the liaison between the prescription drug plan and the patient, facilitating patient engagement and understanding of their treatment plans while directing them to the care and support options. • Act as liaison between members, providers, and pharmacies or care facilities
• Support patients in accessing the appropriate site of care, including enrollment with the required specialty pharmacy
• Provide product-specific financial resources and support patients in completing enrollment requirements
• Coordinate scheduling of specialty medications
• Obtain and process patient order requests

Wage: $16.00 - $17.00 BOE

Schedule: Monday-Friday 1st shift - 40 hours per week
