Position: Occupational Therapist

Status: Full-Time, Part-Time, PRN .

As a Occupational Therapist (OT),you are a qualified licensed clinician with operational responsibilities defined herein who identifies, evaluates, supervises, and treats patients and residents. Where you communicate patient status and needs with patients, families, physicians, and other health professionals. As well as maintaining documentation of services provided in the medical record in accordance with state, federal, and third-party payor requirements.

Job Duties:
  • Screens patients/residents based upon identified needs.
  • Performs evaluations for patients/residents based upon the physician referral.
  • Develops basic treatment plans based on evaluation results including short and long term goals that lead to achievement of patient/resident outcomes.
  • Communicates with supervisor and team members regarding patient progress, problems, and plans.
  • Correctly identifies bills for daily therapy services rendered per approved billing guidelines.
  • Documents the patient's participation, skill provided by therapist, and patient progress on treatment plan goals on required progress notes accurately and timely.
  • Provides patient/resident/family education and training and participates in family conferences for the purposes of discharge planning.

Minimum Qualifications:

  • Bachelors Degree in Occupational Therapy
  • Positive Attitude
  • Ability to complete required in-service training requirements.
  • Must be able to move (walk, bend, stand, sit, push, pull, lift) intermittently throughout the workday.
