Description:

In collaboration with the care team, helps facilitate and collaborate services for the patient while admitted and post discharge. The social worker assesses the patient and family's adjustment to illness and provides psychosocial support to facilitate adjustment to the impact of illness and promote maximum benefit from health care services provided. The social worker provides assistance in transitioning the patient to the most appropriate next level of care and links patients and their families to community resources.

Customers include: patients, families/visitors, physicians, physician assistants, nurse practitioners, case managers, representatives of third-party payers, representative of referring agencies – particularly long-term care, home health agencies and other social service providers – interdepartmental and intradepartmental staff, vendors, and volunteers.

ESSENTIAL FUNCTIONS:
1. Adheres to the principles of caring and expected behaviors outlined in MercyOne Dubuque Medical Center's Model of Caring and Trinity Health's Code of Conduct, fostering a climate where all customers are treated in a manner consistent with MercyOne Dubuque Medical Center's Mission and Values.
2. Maintains a working knowledge of applicable Federal, State and local laws and regulations, Trinity Health’s Organizational Integrity Program, Standards of Conduct, as well as other policies and procedures in order to ensure adherence in a manner that reflects honest, ethical and professional behavior.
3. Assesses, plans, evaluates and coordinates management of patient care including transition/discharge planning and utilization needs in a timely manner.
4. Displays excellent knowledge and practice of psychosocial assessment skills, including social and emotional factors, counseling for long-range health planning and decision-making, community resource planning, crisis intervention and/or short-term therapy, etc.
5. Accurately and completely documents pertinent assessments, interventions and outcomes for patients in accordance with documentation standards, policies, procedures and/or guidelines.
6. Recognizes problems, systematically gathers data, identifies/understands underlying issues, synthesizes complex issues, seeks input from others and makes difficult decisions to formulate appropriate plans of care.
7. Completes behavioral health/substance abuse assessments and makes referrals to inpatient and outpatient services as appropriate.
8. Addresses issues relating to guardianship, adoption, medical decision making, end of life and complex family systems.
9. Collaborates with the multidisciplinary team both internally and externally including physicians and other healthcare disciplines, and outside agencies regarding patient needs, length of stay management and payment issues.
10. Applies knowledge of local, state and federal programs to expedite discharge while being a good steward of healthcare resources.
11. Performs other duties consistent with purpose of job as directed.

EDUCATION, LICENSURE, CERTIFICATION, AND EXPERIENCE REQUIRED:
1. Education, Licensure and Certification
a. An MSW from an accredited school of Social Work required.
b. Social Work licensure strongly preferred.
c. Child and Dependent Adult Abuse Mandatory Reporter Training is required within 6 months
of hire and every 3 years thereafter.

2. Experience:
a. Social Work experience in hospital or other healthcare setting preferred.

3. Special Skills, Competencies, and Aptitudes:
Computer Proficiency: Demonstrated experience and competency with Microsoft Office Suite,
e-mail and a variety of web-based programs and browsers, with the willingness and ability to
update and increase skills as needed to meet the goals of the departments. Ability to use other
software as required to perform the essential functions of the job.
