Job Overview

Your Responsibilities

The Therapist for ACT provides counseling, therapy, and support services to ACT clients within the community. They work collaboratively with clients to address mental health issues, emotional challenges, and behavioral concerns, aiming to improve overall well-being and functioning.

  • Coordinate and provide follow-up contact with clients (ACT services are provided in office, community, and client homes) as needed.  
  • Complete individual case notes within 48 hours of each contact into client’s electronic health records (EHR).
  • Assist with developing and maintaining working relationships with other mental health practitioners, law enforcement officers, hospital personnel, family members, and other interested parties.
  • Participate in case consultation and function as an effective team member and represent the agency in a professional manner.
  • Conduct thorough assessments to evaluate clients’ mental health needs, strengths, and challenges.
  • Use diagnostic tools and techniques to identify mental health disorders and develop treatment plans accordingly.
  • Collaborate with clients to establish realistic objectives and strategies for addressing mental health concerns.
  • Develop personalized treatment plans based on clients’ unique needs, preferences, and goals.
  • Implement interventions to help clients manage symptoms, regulate emotions, and develop coping skills. 
  • Other duties as assigned.

 

What You Need

  • Master's Degree in Social Work, Marriage and Family Therapy, Mental Health counseling or related field
  • Ability to apply the theories and techniques of therapy in the clinical setting in the provision of ongoing services to individuals and families
  • Strong interpersonal and communication skills.
  • Empathy and the ability to build rapport with clients.
  • Excellent problem-solving and critical-thinking skills.
  • Ability to handle crisis situations and work under pressure.
  • Clinical experience providing mental health services, preferably in a community-based or outpatient setting.
  • Experience working with individuals with severe and persistent mental illness.
  • Knowledge of ethical and legal standards governing mental health practice.

  • Full professional license (LISW, LMHC, LMSW or Temp LMHC) 
  • Strong assessment, diagnostic, and treatment planning skills.
  • Proficiency in delivering various therapeutic modalities, such as cognitive-behavioral based therapy, dialectical behavior therapy, or motivational interviewing.
  • Excellent communication, interpersonal, and collaboration skills.
  • Flexibility to adapt to changing caseloads, client needs, and organizational priorities.
  • Experience working with mentally ill clients in a community setting

About the Program

At Hillcrest, the ACT program is a team approach to home and community-based treatment, support, and rehabilitative care for persons with serious mental illness. The ACT program includes a team of treatment providers that offer wraparound services that aim to reduce the number of people who are in and out of the hospital or who are incarcerated or in residential care facilities.  This is achieved by mobilizing mental health and treating people out in the community, in their homes. ACT teams are made up of a Team Lead, nurse practitioner, nurse, therapist, substance abuse counselor, community support specialist, employment specialist and peer support specialist.
