Hospice Nurse Case Manager: Your hometown hospice, Hospice of Dubuque, seeks a Registered Nurse to join our care team. Come be part of delivering the patient-centered care that will remind you why you wanted to be a nurse!

The Hospice Nurse Case Manager:

• functions as a member of the Hospice Interdisciplinary Team,
• assesses the needs of patients and families in a variety of settings and situations,
• participates in the development of each patient’s plan of care,
• performs case management and direct patient care, and
• participates in a night and weekend on-call rotation.

Candidates should possess compassion, sensitivity and dedication, as well as strong problem-solving skills and the ability to work autonomously. RN with minimum one year of nursing experience required. BSN preferred. Candidate must possess excellent interpersonal and organizational skills. Individual must be licensed in Iowa, and willing to obtain Illinois/Wisconsin license if not already licensed there. Competitive wage and benefits package.
