Shift: 3rd – 6:00 p.m. – 6:30 a.m.

Position: Nursing Assistant

Shift: 2nd Shift

As a Nursing Assistant, you will assist the residents with daily living activities such as bathing, dressing, eating, ambulating, and toileting in accordance with the established standards and procedures. Also, providing maintenance and restorative care services to the residents as scheduled such as special hygiene, repositioning, toileting, and equipment servicing such as catheters and wheelchairs. Maintaining related daily care and documents on each resident, and other duties that are necessary.

Position: Medication Manager

Status: PRN

As a Medication Manager, you will be responsible to assists residents with daily living activities with the goal of enhancing each resident’s quality of life, administer medication reminders and/or medication under the direction of a Registered Nurse, provide bathing/showering and personal hygiene assistance, observe and report changes in resident’s condition to Nurse Supervisor, maintain a clean and safe work environment and other duties as assigned.

Position: Physical Therapist

Status: Part-Time

As a Physical Therapist, you will provide and direct physical therapy services to patients. This will include, assessment, treatment, program planning and implementation, related documentation, and communication. You will function under the physician’s orders and will adhere to the applicable principles and practices of physical therapy, facility policies and procedures, and state regulations.

• Conducts screening of patients at regular intervals to determine need for intervention/treatment.
• Develops treatment plans by identifying a problem list, including long and short-term goals and methods to achieve identified goals.
• Coordinates treatment plan with patient, family, and other team members.
• Develops appropriate home or community programming to maintain and enhance the performance of the patient in his/her own environment.
• Performs other duties or special projects as assigned.

Minimum Qualifications:

• Bachelors Degree in Physical Therapy
