Do you enjoy leading others and building collaborative relationships? Area Residential Care is seeking a Residential Supervisor for two of our Residential Services group homes. In this role you will support adults with intellectual disabilities and build relationships with agency staff, outside professionals, and parents/guardians.

Our Supervisors are responsible for managing all components of service delivery. The Residential Supervisor will float between two of our residential locations to facilitate best practices in all levels of service delivery; ensure staff follow approved active treatment, program, and activity plans; and promote adherence to agency policies and procedures. The Supervisor will regularly monitor programs and collaborate to identify growth opportunities. The Supervisor will also assist with setting up on the job training and has to provide feedback to others working.

Job Type: Full-time Generally 11 am-7 pm Monday through Friday with shared on-call responsibilities.

Wage range: $19.30-$23.11

Required qualifications:

Minimum

  • Education: High school diploma or HSED (GED)
  • Skills, Experience: 1 year of supervisory experience required.
  • Must have strong computer skills, including experience with Microsoft Office.
  • Excellent verbal and written communication skills required. Must demonstrate an understanding of basic budgetary information.
  • Valid driver’ license and driving record that meets agency authorized driver criteria.
  • Must be able to lift 20 lbs. regularly, up to 50 lbs. on occasion.

Preferred

Education: AA or BA in Human Services, Management, or Education-related field.

As a Registered Nurse at MercyOne, you will be a collaborative member/leader of the healthcare team and is accountable for practicing both the art and science of nursing, delivering care as prescribed by the ANA Standards of Practice & Professional Performance described below.  In addition, registered nurses are accountable to practice in accordance with standards established/endorsed by MercyOne leadership and by external regulators and accreditation agencies.  The RN actively promotes MercyOne Nursing and the nursing profession and supports advancing practice as defined by the MercyOne Nursing Vision Statement and annual Patient Care Services goals. 

ESSENTIAL FUNCTIONS 

  • Conducts an initial assessment to gather data. 

  • Makes an initial assessment of new admissions within 30 minutes of the patient’s arrival to the unit. 

  • Completes all areas of the admission assessment database within 24 hours of admission.  

  • Identifies patient problems and develops them into an outcome/goal-based plan of care. 

  • Initiates the patient problem list and incorporates relevant medical history into the plan of care. 

  • Analyzes and interprets the assessment data to identify desired patient outcomes/goals. 

  • Determines nursing interventions to achieve those outcomes/goals. 

  • Prioritizes patient care needs/ activities. 

  • Identifies and makes appropriate referrals (i.e., Patient/Family Services, Nutritional Services, Spiritual Care, Diabetic Educator, etc.), and education needs. 

  • Collaborates with other health care disciplines to plan interventions/ patient outcomes/goals. 

  • Implements the outcome/goal- based plan of care. 

  • Assigns and coordinates patient care incorporating patient acuity and the skills, knowledge and abilities of the team members. 

  • Organizes and prioritizes care for the patients within the team. 

  • Delegates tasks appropriately to other team members. 

  • Implements the plan of care including procedures, treatments, physicians orders, nursing orders, education plan and medications.

  • Uses nursing judgment to determine the appropriate frequency and components of reassessment. 

  • Reassesses at regular intervals to determine patient response(s) to interventions (minimum RN reassessment every 24 hours). 

  • Focuses ongoing reassessment on actual and potential problems/needs of the patient including education and discharge needs. 

  • Participates in develop discharge plan.  

  • Documents discharge instructions for patient and/or significant other and/or other care provider.   

  • Evaluates patient progress and revises the outcome/goal-based plan as needed. 

  • Evaluates patient response(s) to interventions. 

  • Communicates relevant clinical information to the team to optimize patient outcomes/goals. 

  • Reports progress toward outcomes/goals and/or changes in condition to other health team members. 

  • Maintains ongoing communication with interdisciplinary team members throughout the shift, at shift change, and transfer of care. 

  • Documents completely, comprehensively and legibly and in accordance with policy. 

  • Demonstrates clinical competence. 

  • Utilizes the Patient Care Policies and Procedure as references to guide nursing practice. 

  • Assures medication safety by following the “5 Rights” of medication administration. 

  • Prioritizes and implements intervention according to patient assessment. 

  • Receives, transcribes and processes and implements physician orders per policy including dating, timing and verifying accuracy. 

  • Maintains communication with the manager/house supervisor and staffing office regarding admissions, unit needs, acuity numbers and significant events.

  • Assists patients with daily activities including dressing, bathing, grooming, toileting and personal hygiene.
  • Performs rehabilitative therapy and activities of daily living.
  • Takes and records vital signs.
  • Assists patients with movement to dining, exercise, and recreation areas.
  • Makes rounds on regular basis; checks patients, changes bed linens, washes patients, empties bed pans, cleans urinals, commodes and other facilities and equipment.
  • Notifies nurses of the need for attention, medication or treatment.
  • Applies devices such as simple dressings, supports, braces, appliances and restraints.
  • Measures and records food and fluid intake.
  • Teaches behavioral skills and protects patients from harm and self-abuse.
  • Monitors and records behaviors related to medical and physical restraints.
  • Follows required protocols for safety and infection control.
  • Performs related tasks as required.

Possess Certified Nursing Assistant certification.

Environmental Service Worker

Performs responsible work in cleaning linens and other cloth products, housekeeping and light manual work in the care and cleaning of patient rooms, halls, work areas, office and other facilities.

Work is performed under the regular supervision of the Environmental Services Director.

Examples of Duties

  • Cleans, mops, and sanitizes resident rooms, toilets, and baths.
  • Cleans windows, windowsills, window treatments, walls, and furniture.
  • Performs special cleaning of rooms, disinfects walls, floors and furnishings.
  • Cleans nursing stations, halls, lobbies, treatment rooms, recreation areas, lounges, and dining areas.
  • Stocks cleaning and linen carts.
  • Empties and cleans waste and trash containers.
  • Washes, dries, counts, folds, and stores clean linens and clothes.
  • Delivers clean linens and clothes to various departments and floors, and ensures the proper amount of linens are delivered by established times.
  • May operate mending, sewing, and marking equipment.
  • Follows prescribed protocols for maintaining privacy and confidentiality of protected health information in accordance with HIPAA standards and County Policies.
  • Follows prescribed protocols for safety and infection control.
  • Acts as Mandatory Reporter as defined in the Code of Iowa.
  • Performs related tasks as required.

 

Minimum Qualifications

High school diploma or GED or experience in building cleaning or laundry operations.

Must pass the prescribed pre-employment physical, abuse registry review, and criminal background check.

Special Qualifications:

Ability to:
Understand and follow oral and written directions.

Physical, Mental, and Environmental Requirements:
Vision abilities required by this job including close vision, distance vision, color vision, peripheral vision, depth perception, and the ability to adjust focus.
Effective verbal speaking and hearing abilities are needed to handle communications with staff, residents, and others.
Requires pushing, pulling, and lifting up to 25 pounds regularly and up to 50 pounds occasionally with assistance.
Requires standing most of the time to perform work responsibilities with regular walking between locations.
Requires ability to walk steps, reaching, squatting, stooping, bending, and sitting, pushing, pulling, twisting, gripping and apply physical intervention holds.
Some exposure to slippery surfaces when floors are wet.
Exposure to chemicals from cleaning compounds and exposure to bodily fluids require the use of personal protective equipment to safe guard against potential biological and chemical hazards.
Work is primarily indoors.
Occasional exposure to outside weather conditions and some outdoor work.

Food Service Worker

Sunnycrest Manor is accepting applications for Food Service Workers who performs routine cleaning and dishwashing, assisting cook and other food service workers with set up and delivery of meals. Position will fill in as need as a cook, and will be compensated at a higher wage for hours worked in Cook position.

Examples of Duties

  • Cleans tables and chairs, kitchen utensils and equipment, stoves, refrigeration storage units, freezers, and kitchen area.
  • Clears food trays and dishes from dining area.
  • Assists with minor food preparation.
  • Follow sanitation protocols.
  • Operate standard cooking equipment/prepare food.
  • Aid and assist residents with meal completion.
  • Stock food and dishes daily.
  • Follow HIPPA and County policies to ensure privacy of residents.
  • Sweeps and mops floors and performs general cleaning duties.
  • Performs related tasks as required.
